Patient Portal
Welcome to the Mid-Cities Allergy & Asthma Center Patient Portal. The Patient Portal provides a secure pathway for patients' to communicate with our Practice and allows convenient access to healthcare related information and services.

New Patients
If you are visiting our office for the first time, please enter your User ID (your email address) and the temporary Password which was sent to your email.  Once you logon to the portal, please complete the new patient registration forms which include the Online Forms (New Patient Medical History and Office Policies Notice) and Patient Agreements (Patient Portal Authorization Form, Notice of Privacy Policies and the Medical Services Financial Agreement). 

For those patients without an email address or who cannot use the Patiant Portal, there are two alternative options for completing the registration process:  
(1) Download the registration forms from our website (www.midcitiesallergy.com => Patient Forms tab => Other Common Forms => Patient Registration - Paper Option) or
(2) Arrive to your scheduled appointment time 30 minutes early in order to complete the new patient registration process.    

Please Note:  You should have received your temporary password via email after you called our office to schedule an appointment.  If you do not have this information, please call our office at (817) 571-6622 for assistance.


Current Patients
Current patients may use the Patient Portal to manage appointments, request a prescription refill, view and print clinical data, communicate with our medical staff, view statements and pay any outstanding balance.  The Patient Portal should not be used for medical emergencies or urgent requests.  If you cannot access the Portal, please call our office for assistance.
